3. Brief introduction and statement that you will be leaving your position and when

4. Provide reason for leaving (optional)
Due to personal circumstances I will be relocating with my family to Miami, FL which in turns dictates my leaving the position and culture I have enjoyed while working here.
5. Express gratitude for the opportunity at the company
I am grateful to [Company’s Name] for providing me the many opportunities I was given as well as the support I have received during my time here.
6. Offer to help with the transition
